1. Specific conditions will be set out in the contract, including (but not limited to) nutrition and
general care, which will be agreed to by both parties, and I understand that by submitting this
application I do not agree to accept a puppy until I have considered and agreed to the terms
of the offer. Yes / No

2. Are you willing to sign a contract agreeing to deliver the dog to us for breeding when she
comes into heat, and 5 days prior to delivering her puppies ? Yes / No

3. Are you willing to take the female to multiple vet appointments for breeding purposes if

4. Do you agree to contact us on “Day One” of every heat cycle? 1st, 2nd, 3rd, etc.
